When is the right time?

Pasco Palliative Care is available during any stage of a chronic illness. The goal of palliative care is to improve symptoms, quality of life and patient satisfaction. Our focus is for you to live your life to the fullest with a healthcare plan tailored to your needs by our staff. We begin the process through a one-on-one consultation that focuses on your life, not just your illness.

Signs you or your loved one needs care

Do any of the signs below affect you or your loved one? If so, you might be eligible for care from Pasco Palliative Care.

• Frequent ER visits or hospital admissions for the same condition
• Chronic conditions or illness such as dementia, COPD, heart disease, liver disease, renal disease, neurological disorder and metastatic cancer
• Functional decline
• Uncontrolled pain and symptoms
• Weight loss
• Increased safety concerns, behavioral concerns, caregiver stress
• Patient is NOT terminal and does NOT qualify for hospice, however has chronic or late stage illness that require complex care and attention
• Chronic home oxygen use
• Limited social and home resources and service support
• Elderly, cognitively impaired, and declining
